How to claim your free PRSI or Medical Card eye test

Give your PPS number when you request an appointment to see if you’re eligible for a free eye test

If you qualify under the Medical Card scheme, you’ll need to come into store to complete an application form

We’ll send the form off to the HSE for authorisation

Once you get the form back, you can book your free eye test
